in accordance with Art. 6 par. 1 letter a) and Art. 7 Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons in the processing of personal data and on the free movement of such data, which repeals Directive 95/46/EC (General Data Protection Regulation)

By confirming and sending this document, I grant the operator Talento s.r.o, , Blumentálska 7881/25, 811 07 Bratislava-Staré Mesto ,  IČO: 55 730 795 (hereinafter referred to as the “operator”) consent to the processing of my personal data for the purpose of arranging employment, obtaining information about suitable jobs and other products and services related to career and personal development. For this purpose, I agree to be included in the operator’s job applicant database and subsequently provide or make available my personal data to potential employers, to the extent of:

• Name, surname, title,
• Date of birth,
• Address of permanent/temporary residence (street, number, city, zip code),
• Contact details (email, phone),
• Labor-legal relationship (current and previous employment/-and to the extent of: employer’s address and job title),
• Achieved education (current, secondary, university),
• Completed courses and trainings,
• Other data from the CV.

This consent to the processing of personal data is a freely given, specific, informed and unambiguous expression of my will, which I grant for a period of 3 years from the date of sending the application for employment. Before granting consent, I was informed of the following facts:

• my personal data will be provided to the recipient (potential employer).
• my personal data, the provision of which is voluntary, will be stored during the period of validity of the consent and will not be processed for any purpose other than the one for which it was obtained.
• as a data subject, I have the right to request from the operator access to my personal data concerning me, the right to their correction or deletion or restriction of processing, the right to object to processing, as well as the right to data portability.
• in a justified case, as the person concerned, I have the right to file a complaint with the supervisory authority, which is the Office for the Protection of Personal Data of the Slovak Republic.
• I acknowledge that there is no automated decision-making, including profiling.

As a data subject, I can exercise my right to revoke my consent at any time, even before the expiration of the period for which this consent was granted, in the following ways:

3. by sending a written request to the address of the operator’s registered office with the text “GDPR – withdrawal of consent” on the envelope.

Withdrawal of consent does not affect the lawfulness of processing based on consent prior to its withdrawal.
